Returning Student-Athletes

All athletes are mandated to complete the yellow physical packet. The forms may be accessed below.

* 2009-10 Informational Letter
* Introductory Letter
* Informed Consent to Participate in Athletics
* Athletic Participation Insurance Form
* Athletic Training/Health Services Waiver and Consent Form
* Pre-Participation Screening Form

The athletic training staff will determine the athlete's clearance. If questionable, the athlete will be referred to the team physician for a physical.

Any athlete with previous injury or illness will be referred to the team physician for final clearance.

Athletes with previous diagnosed conditions (cardio, neuro, etc.) should report to their assigned pre-screen date with an annual report from the specializing physician indicating the athlete's clearance or level of activity.

Student-athletes have the option to purchase insurance through ESU's Consolidated Health Plan. Please click the link for more details.

Make-up Prescreens - 10 a.m. until 2 p.m.

**Any winter sport athlete can arrange for their pre-screen to be completed at an earlier date (Aug. 8, 10, 12, 24). Please contact your respective coach along with the athletic training staff to schedule your appointment.
