Students who request accommodations or academic adjustments are responsible for providing required documentation to the Office of Disability Services and for requesting those accommodations or academic adjustments. East Stroudsburg University will need documentation of the disability that consists of an evaluation by an appropriate professional and describes the current impact of the disability as it relates to the accommodation request.

In order to receive services, students with disabilities must:

  • Identify that they have a disability
  • Submit appropriate documentation
  • Request services

Steps:

  1. Submit the card provided for this purpose in the view book or by contacting the Office of Disability Services at 570-422-3954 (V/TTY), e-mail or in person.
  2. Provide the Office of Disability Services with a copy of appropriate documentation prior to scheduling a conference with the designated disability service provider. (Documentation should be submitted prior to the semester during which the student will request services.)
  3. In order to receive services students (not parents/guardians or other advocates) must meet with the designated disability service provider in the beginning of the first semester in which they will be requesting accommodations. In order to reactivate their status, re-enrolled students must meet with the designated disability service provider.
  4. The disability service provider will review the documentation, interview the student and assess the student's program of study. The disability service providers will use this information to evaluate the request for accommodations/academic adjustments based on their professional judgment.
  5. The disability service provider will review the request for accommodations and complete the accommodation form with the student. Forms are dated and signed by both the student and the disability service provider.
  6. Students will be given copies of the letter that lists approved accommodations or academic adjustments to present to those professors from whom they will be requesting such accommodations or adjustments. After their first semester, students are responsible for picking up their accommodations letters at the beginning of each semester.
  7. Students will have the opportunity to sign a release form giving the faculty member in the Office of Disability Services permission to discuss accommodations or adjustments with classroom faculty who have questions, professional tutors, parents and other relevant individuals.
  8. Students are responsible for meeting with their professors, presenting their letter of introduction (accommodation letter), and discussing their accommodation requests in a timely manner.
  9. It is the student’s responsibility to inform the relevant faculty member and their disability service provider immediately if there is a problem/concern regarding any approved and requested accommodation.
