1. The evaluation must be completed by a licensed and/or certified professional, who is qualified to diagnose the specific disability(ies). The evaluator may not be a relative or member of the student's family.
2. Assessment reports must be typed on letterhead, dated, signed and legible. The assessment report must include testing dates, student's age and grade level, if relevant.
3. A specific diagnosis must be stated.
4. The report should describe the functional limitation(s) related to the diagnosed disability.
5. The report must contain current information (adolescents/adult evaluation).
